It is our office policy to charge for the completion of any forms as follows:

Processing fee of $25 per form

This encompasses completing paperwork for schools, camps, sports participation, weight loss, Family Medical Leave Act (FMLA) claims, disability claims, etc.

Processing forms can take up to 2-3 business days before completed.á
